CLASS Case Manager

Position leading to Full-Time (90–120 day probation period)
Job Duties
• Develop, implement, and monitor Individual Program Plans (IPPs) in accordance with CLASS and HHSC guidelines
• Conduct and coordinate annual, quarterly, and requested Service Planning Team (SPT) meetings
• Monitor waiver and non-waiver services to ensure quality and regulatory compliance
• Complete and submit all required documentation (annuals, revisions, monthly notes, and assessments) by established deadlines, including by the 25th of each month
• Maintain required monthly contact with participants and/or Legally Authorized Representatives (LARs)
• Verify Medicaid eligibility monthly and assist participants in resolving or reinstating coverage when needed
• Connect participants to community resources not available through the waiver program
• Address participant concerns or complaints in a timely and professional manner
• Maintain accurate, professional verbal and written communication with participants, LARs, providers, and team members
• Participate in team meetings, trainings, and professional development activities
• Perform other duties as assigned
About the Role
The CLASS Case Manager coordinates and monitors services for individuals enrolled in the Community Living Assistance and Support Services (CLASS) Program. This role ensures compliance with HHSC guidelines while promoting person-centered planning and quality service delivery.
This position leads to full-time employment following a 90–120 day probationary period.
Requirements
Qualifications
· Bachelor’s degree in Social Work, Human Services, Psychology, Nursing, or related field (preferred)
· Minimum of four (4) years of Case Management experience (required)
· Experience working with individuals with disabilities (preferred)
· Knowledge of the CLASS Waiver Program and HHSC regulations (preferred)
· Strong documentation and organizational skills
· Ability to travel within assigned catchment area
· Valid Texas driver’s license and reliable transportation
Skills & Competencies
· Strong communication and interpersonal skills
· Ability to work independently and effectively manage a caseload
· Knowledge of Medicaid waiver programs (preferred)
· Proficiency in documentation and compliance standards
· Proficient in Microsoft Word, Adobe, and DocuSign
· Basic math proficiency
· Ability to provide clear directions, instructions, and accurate information
· Commitment to exceptional participant service
